Participation Waiver for Minors (Under 18 Years of Age)
Please read this document carefully before signing.

ASSUMPTION OF RISK
I understand and acknowledge that participation in basketball activities, including games, practices, tournaments, conditioning, travel, and related events, involves inherent risks of injury, including but not limited to sprains, fractures, concussions, head injuries, permanent disability, illness, paralysis, and death. I voluntarily assume all risks associated with my child’s participation in Above The Rim 3v3 activities in the State of New York.

RELEASE OF LIABILITY
I hereby release, waive, discharge, and covenant not to sue Above The Rim 3v3, its owners, organizers, coaches, referees, volunteers, sponsors, affiliates, employees, agents, and facility owners from any and all liability, claims, demands, damages, costs, expenses, or causes of action arising out of or related to my child’s participation in league activities, including injuries sustained on or off the court, except where prohibited by applicable New York law.

MEDICAL AUTHORIZATION
In the event of an emergency, I authorize Above The Rim 3v3 staff, representatives, or emergency personnel to obtain medical treatment for my child, including emergency transportation if deemed necessary. I understand and agree that I am solely responsible for any medical expenses incurred as a result of such treatment.

PHOTO & VIDEO RELEASE
I grant permission to Above The Rim 3v3 to use photographs, videos, livestreams, recordings, or other media containing my child’s image or likeness for promotional, marketing, social media, website, and advertising purposes without compensation. Participation is optional, and parents/guardians may submit a written request to opt out of media usage prior to or during the season.

SPORTSMANSHIP & CODE OF CONDUCT
Players and parents/guardians are expected to maintain the highest standards of sportsmanship and respectful conduct. Foul language, threats, verbal abuse, fighting, harassment, dangerous behavior, or physical aggression toward players, coaches, referees, staff, or spectators may result in immediate suspension or permanent removal from the league without refund.

RULES & COMPLIANCE
I agree that my child and family will comply with all league rules, policies, schedules, safety requirements, and official communications issued by Above The Rim 3v3. Failure to comply may result in disciplinary action, suspension, or removal from participation.

GOVERNING LAW
This agreement shall be governed by and interpreted under the laws of the State of New York. Any disputes arising from participation in Above The Rim 3v3 activities shall be subject to the jurisdiction of the courts located in the State of New York.

ARBITRATION AGREEMENT
Any dispute, claim, or controversy arising out of or relating to participation in Above The Rim 3v3 activities, including but not limited to personal injury claims, property damage claims, negligence claims, enforcement of this waiver, or disputes involving league operations, shall be resolved exclusively through binding arbitration in the State of New York.

The parties agree to waive their right to file a lawsuit in court or to participate in a jury trial, except were prohibited by applicable law. Arbitration shall be conducted in accordance with the rules of the American Arbitration Association (AAA) or another mutually agreed arbitration service.

The parent/legal guardian further agrees that any arbitration proceeding shall be conducted on an individual basis only, and not as part of a class action or collective proceeding.

INDEMNIFICATION & HOLD HARMLESS
The parent/legal guardian agrees to indemnify, defend, and hold harmless Above The Rim 3v3, its owners, organizers, coaches, referees, volunteers, sponsors, affiliates, employees, agents, and facility owners from and against any and all claims, demands, actions, liabilities, damages, losses, judgments, costs, or expenses, including reasonable attorney’s fees, arising out of or related to:
(a) the minor participant’s actions or conduct during league activities;
(b) any breach of this agreement by the parent/legal guardian or participant; or
(c) any claims brought by or on behalf of the participant, parent/legal guardian, or third parties related to participation in Above The Rim 3v3 activities.

This indemnification provision shall survive the participants’ involvement with the league and the execution of this agreement.

By signing this agreement, I acknowledge that I have read, understood, and voluntarily accepted the risks associated with concussion and head injuries related to participation in basketball activities.
